Required field names:
 1. form_name (Hidden)
 2. mail_to   (Hidden)
 3. action    (Hidden)
 4. username
 5. email
 6. subject
 7. message
You can ad as many fields as you want, but these six fields are checked by the program to see if they have been filled out.

form_name = The name of the form as you want it displayed in the program.

mail_to = Who you want the forms info to goto. i.e.;  sales@yourdot.com
Note:
Keep in mind, that the order in which you have your form fields displayed here in the form, is the same order in which it will be displayed later when the program processes the form and displays the results.
